It highlights enrollment in other health programs, cost distribution between Medicaid and Medicare, and health coordinator assignments. By centralizing and visualizing this information, the dashboard has improved program effectiveness and efficiency by over 75%, enabling faster, more informed decision-making and better care coordination for a vulnerable population.
